Macular Degeneration

Macular degeneration is a leading cause of vision loss among older adults. Although it can develop gradually without noticeable symptoms, regular comprehensive eye exams can help detect early changes and protect long-term eye health. Understanding the condition, its warning signs, and available treatment options is an important step toward preserving your vision.

What Is Macular Degeneration?

Age-related macular degeneration (AMD) is an eye condition that affects the macula, the central portion of the retina responsible for sharp, detailed vision. Damage to this area can make everyday activities such as reading, driving, and recognizing faces more difficult. Macular degeneration primarily affects central vision. Peripheral vision usually remains intact, meaning the condition does not typically cause complete blindness.

There are two main types of age-related macular degeneration:

  • Dry Macular Degeneration: The most common form, dry AMD involves gradual changes to the macula, including deposits called drusen and thinning of retinal tissue. It can progress through early, intermediate, and late stages.

  • Wet Macular Degeneration: This less common but more aggressive form develops when abnormal blood vessels grow beneath the retina and leak fluid or blood. Wet AMD can cause rapid central vision loss and requires prompt treatment.

Who Is at Risk for Macular Degeneration?

The risk of developing macular degeneration increases with age, particularly after 55. Additional risk factors include family history, smoking, high blood pressure, and elevated cholesterol. While some risk factors cannot be controlled, avoiding smoking, maintaining cardiovascular health, exercising regularly, and eating a balanced diet may help reduce the risk of AMD progression.

Common Signs and Symptoms

Early macular degeneration often develops without noticeable symptoms. As the condition progresses, patients may experience:

  • Blurry or distorted central vision

  • Difficulty reading or recognizing faces

  • Needing brighter lighting for detailed tasks

  • Straight lines appearing wavy or crooked

  • Dark or blank spots in the center of vision

Sudden changes in central vision, particularly new distortion or dark spots, require urgent evaluation by an eye care professional.

The Importance of Routine Eye Exams for Early Detection

Regular comprehensive eye exams are essential for detecting macular degeneration before significant vision loss occurs. During a dilated eye exam, our eye doctor can examine the retina for drusen and other abnormalities. Additional testing, such as optical coherence tomography (OCT), may help identify structural changes and monitor disease progression. Early detection allows for closer monitoring, appropriate treatment recommendations, and timely referrals when necessary.

Macular Degeneration Management and Treatment Options

Although there is currently no cure for macular degeneration, several approaches can help manage the condition and reduce the risk of further vision loss.

  • Lifestyle Modifications: Quitting smoking, maintaining a nutritious diet, and managing cardiovascular risk factors may support retinal health.

  • AREDS2 Supplements: Specific vitamin and mineral formulations may reduce the risk of progression to advanced AMD in certain patients, particularly those with intermediate disease.

  • Valeda Light Therapy: The FDA-authorized Valeda Light Delivery System uses photobiomodulation, delivering specific wavelengths of light to improve visual acuity in select patients with dry AMD.

  • Anti-VEGF Injections: These medications help control abnormal blood vessel growth and leakage associated with wet AMD.
  • Geographic Atrophy Treatment: For certain patients with advanced dry AMD, specialized injections may slow the enlargement of damaged retinal areas but cannot restore lost vision.

Treatment recommendations depend on the type and stage of macular degeneration, retinal findings, and individual vision needs.
